Requirements

Individuals

Individuals seeking rebuilt inspection must have the following:

 

  • Certificate of Title (Original copy only)
  • Copy of owner’s driver's license
  • Photos of the ENTIRE vehicle inside and out BEFORE the repairs took place
  • Original copy of receipts for all major component parts replaced. (See below for list of major component parts)
  • Original copy of body shop work order receipt (if applicable)
  • Power of Attorney if inspection documents are not being signed by the owner 
  • Completed statement of builder form
  • Completed and signed application for certificate of motor vehicle title form

Dealers

Dealers seeking rebuilt inspection must have the following:

 

  • Certificate of Title (Original copy only)
  • Copy of driver's license of any officer of the dealer 
  • Photos of the ENTIRE vehicle inside and out BEFORE the repairs took place
  • Original copy of receipts for all major component parts replaced (see below for list of major component parts)
  • Original copy of body shop work order receipt (if applicable)
  • Power of Attorney if inspection documents are being signed by a person who is not an officer of the dealer
  • Completed statement of builder form
  • Completed and signed application for certificate of motor vehicle title form

 

*Major component parts for motor vehicles: Fender, hood, bumper, cowl assembly, rear quarter panel, trunk lid, door, decklid, floor pan, engine, frame, transmission, catalytic converter, and airbag*
